Impotence, or sexual impotence, is a condition when a man is unable to perform sexual intercourse. Sexual function is ensured by congenital and acquired factors, such as hormones, individual experience, psychosocial influences, etc. Neuroendocrine factors associated with the activity of the brain and endocrine glands provide the severity and degree of sexual desire. An erection, the most characteristic manifestation of male sexuality, allows for intimacy during sexual intercourse. Ejaculation provides the main biological purpose of sexual activity - the release of semen.

However, a huge role in the implementation of normal sexual function in a person is played by the psyche as a whole, which determines the direction of sexual desire, the choice of a sexual object and the form of sexual activity specific to a person, taking into account a whole complex of moral and ethical attitudes.

Impotence can arise from the interaction of somatic, personal and microsocial influences. In most cases, we should talk about the complex nature of impotence, which is caused by dysfunctions of the genital organs, endocrine system, peripheral nerves and higher parts of the brain.

Typically, sexual potency disorders are not an independent disease, but develop and exist as a painful manifestation accompanying the underlying disease. Impotence can occur with endocrine disorders caused by damage to the deep structures of the brain or individual endocrine glands; it can be caused by erectile dysfunction due to damage to the spinal cord, nerve fibers and plexuses innervating the penis, or diseases and injuries of the penis.

According to many sex therapists, most cases of impotence are associated with disorders of higher nervous activity, including congenital and acquired neuropsychiatric disorders, neurosis, neurotic state in various diseases, depression, etc. In humans, the sexual sphere turns out to be very vulnerable to mental trauma. Therefore, with prolonged exposure to factors that seemingly do not directly affect the sexual sphere, along with general neurotic disorders, sexual disorders often occur.

Potency disorders can also be caused by certain physical factors, such as atherosclerosis, hypertension, diabetes, heart disease, alcohol and drug abuse, etc. In addition, some medications used to treat other diseases can cause impotence.

Treatment for impotence depends on the cause of its occurrence. If the cause is a physical disease, then treatment for this disease is necessary. If impotence is caused by psychological factors, then consultation with a psychologist or psychotherapist may be required. In some cases, medications such as phosphodiesterase type 5 (PDE5) inhibitors may be prescribed to improve blood flow to the penis and help achieve and maintain an erection.
