Medical documentation is a set of documents that contain medical and statistical information about the health status of individuals, various population groups, the volume, content and quality of medical care and the activities of medical institutions.

In the USSR, medical documentation was mandatory, uniform and unified. It was used for health care management and planning of public health activities.

Medical documentation includes medical histories, outpatient records, reports from medical institutions, certificates of examination of temporary disability and other documents.

Maintaining accurate and complete medical records is extremely important for the provision of quality medical care, ensuring continuity of treatment, as well as for statistical recording and analysis of public health data.
