Dosage forms - solution for infusion 250 mg
Composition - Active substance - dobutamine.

Indications for use - Acute heart failure: acute myocardial infarction, cardiogenic shock, exacerbation of chronic heart failure, chronic heart failure (as a temporary aid).

Contraindications - Hypersensitivity, idiopathic hypertrophic subaortic stenosis, pregnancy (restrictions on use), lactation.

Side effect - Headache; tachycardia, atrial fibrillation, pain in the heart and chest, palpitations, shortness of breath, increased blood pressure, hypotension, hypokalemia, inhibition of platelet aggregation (with long-term use), petechial bleeding, phlebitis; nausea; allergic reactions (skin rash, fever, bronchospasm, eosinophilia, polyuria); skin necrosis at the injection site.

Interaction - There is a more pronounced decrease in OPSS and an increase in cardiac output when combined with sodium nitroprusside than when administered separately. Beta-blockers reduce the positive inotropic effect, and an increase in peripheral vascular resistance is possible. Pharmaceutically incompatible with ethanol, solutions containing sodium bisulfite, ethanol, sodium bicarbonate and other alkaline solutions.

Overdose - Symptoms: headache, shortness of breath, nausea, vomiting, anorexia, tremor, anxiety, palpitations, angina or cardialgia, tachyarrhythmia, severe hypertension, myocardial ischemia. Treatment: stopping the administration of the drug, providing ventilation and blood oxygenation.

Special instructions - During treatment, constant monitoring of blood pressure, ventricular filling pressure, central venous pressure, pulmonary artery pressure, heart rate, ECG, stroke volume, body temperature and diuresis is required. Hypovolemia should be compensated for before administration. With cardiac tamponade, valvular aortic stenosis, subaortic stenosis, a paradoxical reaction (decreased cardiac output) may develop.
