Pulpitis is inflammation of the dental pulp, manifested by attacks of pain.

Etiology. Microflora of the carious cavity, chemical, mechanical and thermal injuries.

Pathogenesis is associated with the progression of the carious process and the inflammatory response to microflora and pulp trauma. Infection through the apical foramen is possible in periodontal diseases.

Symptoms, course. Spontaneous paroxysmal pain, especially at night, provoked by mechanical, chemical and temperature stimuli. Irradiation of pain along the corresponding branch of the trigeminal nerve is characteristic.

In serous forms, at the beginning, attacks of aching pain are infrequent and short-lived. As they progress, they become more frequent and lengthen. With purulent pulpitis, the attacks are long, pulsating. Cold causes pain in serous pulpitis, and relieves it in purulent pulpitis. In chronic forms, attacks occur during exacerbations.

The diagnosis is made by characteristic complaints, carious cavity and probing of its bottom. Percussion is usually painful in purulent and chronic forms.

Treatment is aimed at relieving inflammation and pain. In serous forms, it is possible to preserve the pulp in young people: excision of the affected dentin, treatment with antibiotics, calcium hydroxide and filling.

Surgical treatment - removal of the entire or coronal pulp under anesthesia or after devitalization.

Emergency care for acute serous forms: irrigation with a warm anesthetic, a swab with corticosteroids into the cavity. In case of purulent - opening of the tooth cavity. Analgesics are prescribed.

Forecast. Without treatment, pulp necrosis and transition of the process to the peri-apical tissues. With treatment, the outcome is favorable.

Pulpitis is an inflammation of the dental tissue located inside the dental cavity - the dental pulp. The coronal part of the pulp passes into the cement and fills the oral cavity, and the root part into the dental root canal, its continuation is the periapical tissue. Thus, most forms of pulpitis occur in the root part of the pulp, that is, in its inner part, the inner part of which is located in the area of ​​the neurovascular bundle. But sometimes the coronal, that is, the outer part, is also affected, and then they talk about peripulpal, or periodontal pulpitis. This causes increased temperature inside the oral cavity, which is why pulpitis is quickly complicated by inflammation of the oral mucosa.

Only a specialist, dental therapist or dentist can determine the presence of pulpitis, provided that the existing problem is a continuation of caries. The doctor must examine the affected area, check the sensitivity of dental tissues for irritants using special instruments, calculate the size of the dental crack and find out its depth.
