The divergence of the walls occurs as a result of a violation of the continuity of the shell and the appearance of a gap in it, into which a foreign body passes, which was closed in the shell before the rupture, or due to the expansion of its narrow passages, or a violation of continuity in them. If this happens in such a way that the body that has penetrated the gap, passing through, reaches the testicles, then such damage is called hydrocele, or clubroot, and everything else is called by a common name. Most often, hydrocele of the testicle, dilatation of its veins, hardening of the testicle and hardening of the scrotum occurs with a hernia of the omentum; sometimes it happens that the above-mentioned openings are enlarged due to the weakness of the walls of the abdomen, or the places adjacent to them are ruptured under the action of some liquid, gluing, wetting or relaxing, or this is facilitated by a cry, a sudden movement or fall, or the retention of a moving seed, which is not allowing to come out, or the fact that the woman lay on top of the man, or fatigue of breathing during intercourse, especially after overflow. The same applies to copulation after overeating or the accumulation of wind or feces in the stomach, as a result of which either the omentum, or the barrier, or both together, or the intestine, especially the cecum, descends, since it hangs freely and is not tied. Or liquids are poured into these holes, driven by nature or arising near them, due to their coldness and the transformation of blood into water, and often a special shell is formed for this liquid. It happens that such a liquid turns out to be blood or moisture, bloody and similar to wine grounds, when the cause of the disease is a blow, or a fall, or blowing winds. In some cases, testicular hydrocele develops after iron treatment, and sometimes excess meat grows there. Often the scrotum thickens or hardens due to a tumor, or becomes fat and becomes similar to hydrocele, and then it is called “meaty”, and sometimes it occurs in the groin. It happens that the vessels in the scrotum become swollen, and this disease is called “dropsy of the testicle from varicose veins,” or it weakens greatly, without divergence, and lengthens and also resembles hydrocele of the testicle. It also happens that the discrepancy occurs above the testicles and occurs in the groin or even higher, as well as in the navel or above the navel, or in the ureters. Dehiscence above the navel is rare in comparison with other types of it, because this place is supported by muscles, and the tissues under it reach the ends of the muscles. It sometimes happens that the navel protrudes, and this also falls into the category of discrepancies. The separation of the walls above the navel is accompanied by bad symptoms, although it increases slightly and does not cause pain at first. The fact is that the insides that rush into the gap are the small intestines, which crowd and press on each other, so that the feces are locked up and come out in vomit; such a disease is something like ileus, and it also worries and depresses the patient. But the discrepancy that occurs below the navel is more susceptible to expansion and is more likely to increase without causing pain at first. And know that clubroot of the intestines and omentum is a strong and severe disease, although the hernia is small, and water clubroot is a mild disease, although it is
Signs. As for the general sign of discrepancy, this is a swelling, clear and felt between the inner lining and the walls of the abdomen, which becomes more obvious when moving and holding the breath. In case of a hernia due to the expansion of the passage, the sign is that it appears little by little in the scrotum, regardless of sudden movements, screaming, etc., and this is testicular hydrocele. Dehiscence above the navel is necessarily due to a rupture, and drying it out does not help. A sign of an intestinal hernia passing through the gap is that it quickly returns when the patient lies on his back, as well as a rumbling sensation, especially with pressure. A hernia of the omentum and membrane is indicated by the fact that it arises slowly, rather in the depths, and stands erect in place. With such dropsy of the testicle, rumbling is not felt and its volume in depth is most often small, but sometimes it all comes out and turns out to be large in size. It is difficult to cure and is not the same as intestinal clubroot, and when palpated it differs from intestinal, water and wind clubroot. The intestinal clubroot and omental clubroot return with greater difficulty than the wind clubroot.
The water clubroot is recognized by touch, as well as by the tension of the scrotum, its shine and smoothness; such a keel also cannot be returned and introduced back. The wind clubroot is known, because the swelling from the winds is quite obvious. Wind clubroot returns without much pressure or pain and sometimes returns immediately. Lying supine does not force it to return faster than another position, and the judgment about it when lying down and not when lying down is similar, because heaviness and sliding are not inherent in it, but with an intestinal hernia it is different, and when the patient lies supine, the reduction is a little easier. Wind clubroot sometimes causes severe pain, as it stretches the scrotum and puts pressure on the testicles. The sign of clubroot is that it is on the scrotum itself, but not inside it, and there is hardening, thickening and change in the shape of the scrotum. Sometimes clubroot occurs due to the petrification of a solid tumor and is called luris. As for hydrocele of the testicle from dilation of the veins, it is recognized by the filled vessels and their bulges, similar to bunches of grapes, with relaxation of the testicles and the inability to press them in and move; with such dropsy of the testicle in the arteries, squeezing with the fingers breaks the clots, whereas if the dropsy is not in them, but in the veins feeding these organs, squeezing does not break them.
Treatment. As for the general regime for those suffering from abdominal wall discrepancy, they should avoid overfilling, avoid strong movements, jumping, do not stand on their feet at once and do not copulate; Such actions are most harmful after overflow. They should avoid strong foods, not drink a lot of water, and avoid anything that relaxes them, even the bath. When the patient has eaten, he must lie on his back, and when sitting, have the hernia closed; This is especially necessary during copulation, and copulation should take place on a light stomach. Let it be known that the goal in the treatment of divergence of the abdominal walls is to heal the rupture, if possible, or to protect it so that it does not enlarge, to dry out what has weakened and expanded, and to return to the place of what has subsided, if it is the omentum or intestine. The doctor also ensures the resorption of what has accumulated in the gap, if it is water or winds, and prevents the formation of matter that stretches the gap. And if it does not resolve, then measures are taken to remove it.
Further, healing the gap or protecting it so that it does not increase is carried out with strengthening and adhesive medicines, which have an astringent force; the smaller the gap, the easier it is to close it; sometimes they resort to cauterization. And drying is carried out with absorbable medications, and at the same time they often also resort to cauterization. The return of what has dropped is achieved by tightening and bandaging, and the resorption of what has accumulated is carried out using bandages for dropsy and the like. The formation of matter is prevented by emptying and balancing the nutrition, and it is removed by strong diaphoretic drugs or by acting with iron.
Treatment of intestinal hernia and omentum. If the insides have descended into the scrotum, then they can be returned, although this is more difficult than returning them with a hernia at the top; in the latter case, this can be easily done by placing the patient on his back and applying slight pressure with his hand. If the discrepancy is large, then they begin to dry out what has expanded due to the liquid, compress the burst walls and try to heal them. If the prolapse cannot be returned, then the patient is placed in hot water and a bandage with emollient medicines is applied to the hernia or steamed with hot rags until the prolapse returns to its place. Then they tighten the hernia, applying collecting medicines to it, and leave it like that for several days, with the patient lying on his back; tightening is done with square bandages and bandages adapted to connect the edges of the gap. Often, in this position, cauterization is used after tightening, but round bandages are not used, because they widen the gap.
If the discrepancy is very large, then splicing cannot be avoided, but you should absolutely not use iron with such a discrepancy. Medicines for drinking, from which a patient with such a gap benefits, are Shajazaniyya, a decoction of cypress cones, especially if Shajazaniyya was diluted in it, as well as Kammuni; medicinal dressings used for discrepancy should be applied after connecting the edges of the gap and pulling the testicles upward, after the return of what has dropped is completed. Then they apply one of those dressings that are made from juniper berries, cypress cones and cypress leaves - these are the main dressings, and everyone agrees that they are very useful, as well as from bdelium, tragacanth, gum arabic, fish glue, animal skin glue , mistletoe, dried mushrooms, crayfish meat, roses along with their cups, as well as from all astringent medicines, from mastic, from dry myrtle, from shelled mung bean, from carcass, from the leaves of Meccan hudad, from Yemenite alum, from sumac, from tamarisk fruits , from red ocher, from yarrow santolina, from samanjan sabur, from myrrh.
Description of the bandage. They take ushshak, incense, samanjan sabur and mistletoe - three dirhams each, bdelliyya - two dirhams, akakiyi and anzarut - each dirham; all this is crushed in a mortar and moistened with vinegar at the beginning of the night. Then, in the morning, the composition is ground with a small amount of juniper berries, a piece of cotton wool is soaked in it, and it is placed on the sore spot and tied.
Light bandage. They take equal parts of mastic, anzarut and incense, bind it all with liquid glue dissolved in raisin nabiz, spread it on paper and tie it. They also use sabur with glue and incense. Another medicine: take equal parts of cypress cones, incense, acacia, pomegranate flowers, anzarutat dragon's blood, myrrh, khudad, juniper berries, finely ground, mixed with gum and kept on the testicle or any place where the hernia is until the medicine disappears.
An excellent bandage that sometimes heals hernias in children. They take incense peels - ten dirhams and unripe galls - five dirhams, boil them strongly with astringent wine in the amount of five ukiy, then return the intestines to the top, pour cold water on the sore spot and apply a bandage, which is untied no more than once a week or ten days.
Wonderful bandage. Take equal parts of mastic, incense peels, cypress cones, fish glue and anzarut, dissolve the glue in vinegar, bind medicines with it and prepare a bandage. Sometimes a dressing made from pomegranate flowers or flea plantain and wild iris root is enough for children. Sometimes it is enough to apply a bandage of water lentils, and this is one of the varieties of duckweed, or it is enough to lubricate the hernia with bdelium, diluted in wine with jasmine oil or with beaver stream, especially when the hernia is water. It is often sufficient to seal the gap with barley oatmeal.
Treatment of water hernia. Sometimes the fluid is drained by gradual needling, and often it is removed using drainage dressings. After this, sometimes they burn it with iron or sharp medicines, tightening the area of the membrane adjacent to the hernia, so that it narrows and moisture does not go down. As for the puncture and incision, the testicles should be raised upward and significantly removed from the scrotum; first lubricate the patient's pubis with nura and strip it of hair. The patient lies supine on a table or bench, and a servant is placed to his right, who pulls his penis upward, after which a puncture is made with a wide lancet; beware of piercing at the seam and take it to the right or to the left; open diagonally from the seam and try to drain and remove all the liquid. And after the puncture you are given a choice - if you want, let the liquid return and fill the cavity again after a while, and then repeat the treatment, if you wish, by puncture, and if you want, by cauterization. Cauterization is done in this way: take a thin, curved piece of iron and prick it in the same way as a cauterizer is heated. Then bandage the testicles as far as possible from the cauterized areas and turn the cauterizer on the scrotum so that it does not fall on the testicles, but falls on the scrotum and baritown, tightening and wrinkling them, and the water would no longer enter there; The wider the cauterizer grips, the better. After that, you treat the scabs and heal the wound. Sometimes they cut off a little bit of baritune, and then cauterize it and apply astringents to the gap; the patient is prohibited from drinking water. As for bandages for water hernia, they are of the same kind as bandages for dropsy and diseases of the spleen. For example, they take larkspur and cumin and bind them with seeded raisins, binding by pounding; the composition becomes like an ointment, and a bandage is made from it. Or they take pepper, laurel drupes, bavrac, wax and old olive oil, prepare an ointment from this and apply it to the hernia. Also: they take oak ash, mix it with olive oil, thickened by boiling, and make a bandage from it; she helps a lot. Also: they take soda - thirty dirhams, wax - six ukiy, olive oil - six ukiy, pepper - one hundred grains, laurel drupes - eighty pieces and make a tight-fitting bandage from it. Arabian bdelium with human saliva often dissolves clubroot in children.
Treatment of wind hernia. The regime in this case is to avoid swelling of vegetables and grains, excessive overfilling, which leads to rumbling and indigestion, and also not to drink diluted wine and unripe wine, which causes swelling. The patient is given wind-dispersing medicines, for example, Kammuni, Shajazaniyya and Atriful al-Kabir, all with a decoction of galanga.
A good medicine for such patients: take equal parts of dry rue leaves, shenos, cumin, azhgon, twig seeds, bavrak, mint, and also dodder - as much as anything else; medicines are combined with honey. Bandages are also applied from rue, cumin, twig, mint, calamus, laurel drupes, marjoram and maya, and the oils that are rubbed into such a hernia should be bush oil, jasmine oil and, especially, spikenard oil; poultices are made from the often already mentioned wind-dispersing substances. When the pain intensifies, use healing candles made of honey, soda, sagapen, opopanax, cumin, rue seeds, rue leaves and beaver stream, using all these substances or some of them, depending on the need.
Meat clubroot and clubroot with varicose veins. They are treated in the same way as solid tumors are treated. Often, for carina with varicose veins, it is enough to rub in letter ointment, basilikun patch, emollient fats and animal bone marrow.
