Bailey Commissurotomy

Commissurota is a surgical method for treating severe mitral valve stenosis, characterized by a low pressure gradient caused by hypertrophy of the papillary muscles and mitral commissure (narrowings).

When performing commissurotomies, the standard Bailey principle is used, in which the skin incision is made in an arcuate manner from one lateral end of the sternocleidomastoid muscle to the other. If necessary, a second one is made parallel to the first skin incision, which runs from the upper edge of the sternum towards the collarbone. The skin incision should be at a sufficient distance from the mitral valve annulus. The proximal section of the cutaneous-subcutaneous excision arc is moved proximally until the free edge is at the left sternocleidoid process.
