Compound

The active substance is alprostadil.

Indications for use

Erectile dysfunction of neurogenic, vascular, psychogenic or mixed etiology; conducting a pharmacological test as part of a set of diagnostic tests for erectile dysfunction, chronic obliterating diseases of the arteries of the extremities, atherosclerosis of the arteries of the extremities, diabetic angiopathy, vasculitis, peripheral circulatory disorders, congenital ductus-dependent heart defects in newborns.

Contraindications

Increased sensitivity, abnormalities of the penis; sickle cell anemia, thrombocytopenia, polycythemia, multiple myeloma, venous thrombosis, increased blood viscosity, acute myocardial infarction, angina pectoris, arrhythmias, broncho-obstructive syndrome, pulmonary edema, liver dysfunction, severe cerebral vascular lesions, pregnancy, lactation, age under 18 years and over 75 years of age.

Side effect

Penile pain, penile swelling, penile rash, prolonged or painful erection and priapism, fibrosis, balanitis, urethral bleeding, penile heat, numbness, fungal infection, irritation, phimosis, prurigo, erythema, ejaculation disorder , swelling of the testicles, tension in the scrotum, increased urination, urinary incontinence; peripheral vascular disorders, blood pressure fluctuations, supraventricular extrasystole; dizziness, headache, hypoesthesia; weakness in the buttocks, localized pain (in the buttocks, legs, genitals, abdomen), pain in the pelvis, lower back, flu-like syndrome, hematomas, hemorrhages, inflammation, nausea, vomiting, diarrhea, swelling, itching at the injection site.

Interaction

Enhances the effect of anticoagulants, antihypertensive drugs, antiplatelet agents and vasodilators.

Overdose

Symptoms: hypotension, pain in the penis, prolonged erection (more than 6 hours), irreversible deterioration of erectile function. Treatment is symptomatic.

special instructions

Dose selection should be carried out under the supervision of a physician, under monitoring the state of the cardiovascular system and the duration of erections. The patient should inform the doctor about all cases of erection lasting more than 6 hours during treatment. During treatment, an adequate method of contraception should be used by the female partner of childbearing age. The female partner may experience a burning and itching sensation in the vagina. The number of adverse reactions increases depending on the concentration and speed of infusion. Increases in infusion volume should be limited in patients with severe heart failure. Caution must be exercised in patients with arterial hypertension; in elderly patients with insulin-dependent diabetes mellitus, when prescribed to newborns in case of bradypnea, arterial hypotension, tachycardia and hyperthermia. During treatment, it is necessary to monitor hemodynamic parameters, acid-base status, biochemical parameters and parameters of the blood coagulation system. A prerequisite for use in newborns is the availability of artificial ventilation. The drug should be used under close medical supervision. Dynamic monitoring of the patient's condition is necessary, if necessary - monitoring body weight, fluid balance, measuring central venous pressure or echocardiography.
