Perinephric Abscess

Perinephric Abscess is a collection of pus around the kidney that is usually secondary to pyonephrosis.

Pyonephrosis is the accumulation of pus in the kidney due to obstruction (blockage) of the urinary tract. This leads to the development of infection and inflammation in the kidney. If the infection spreads beyond the kidney, an abscess may form around the kidney in the perinephric (around the kidney) space.

The main symptoms of a perinephric abscess include fever, chills, low back pain, nausea, and vomiting. With palpation, you can feel a painful formation in the kidney area. Diagnosis is based on blood tests (increased ESR, leukocytosis), urine, and imaging (ultrasound, CT, MRI).

Treatment includes drainage of pus surgically or by puncture, antibiotics, and correction of urinary tract disorders. The prognosis depends on timely detection and adequate treatment. Untreated perinephric abscess can lead to sepsis, kidney failure, and death.

It develops due to a urinary tract infection such as pyelonephritis. If the infection spreads from the renal pelvis to surrounding kidney tissue, an abscess may form.

Perinephric abscess is usually caused by bacteria, especially E. coli, Klebsiella, Proteus. Fungal infections are less common. Risk factors are urolithiasis, urinary tract abnormalities, diabetes mellitus, and immunodeficiency.

Clinical manifestations include fever, chills, lumbar pain, and dysuria. Nausea, vomiting, decreased appetite, and weight loss may occur. Diagnosis is based on blood tests, urine tests, and imaging (ultrasound, CT, MRI). Treatment is primarily antibiotics, sometimes requiring drainage or surgery.

Complications include sepsis, spread of infection to other organs. Therefore, timely treatment of perinephric abscess is important. The prognosis with adequate therapy is generally favorable.
