Hematoma Extrapleural

Extrapleural hematoma is an accumulation of blood between the parietal layer of the pleura and the chest wall. It usually occurs as a result of rib injury or after chest surgery.

The causes of extrapleural hematoma can be:

  1. Fractures of ribs with damage to intercostal vessels
  2. Blunt chest trauma
  3. Puncture of the pleural cavity
  4. Operations on the chest organs

Clinical manifestations:

  1. Chest pain that gets worse with breathing and coughing
  2. Dyspnea
  3. Palpable soft tissue thickening of the chest wall
  4. Crepitus on palpation due to blood clots

Diagnosis is based on history, clinical picture and imaging (radiography, ultrasound, CT).

Treatment depending on the size of the hematoma: puncture and drainage; with a large volume - thoracotomy and removal of clots.

Thus, extrapleural hematoma is a dangerous complication of chest injuries, requiring timely diagnosis and adequate treatment.
