Extrapleural hematoma is an accumulation of blood between the parietal layer of the pleura and the chest wall. It usually occurs as a result of rib injury or after chest surgery.
The causes of extrapleural hematoma can be:
- Fractures of ribs with damage to intercostal vessels
- Blunt chest trauma
- Puncture of the pleural cavity
- Operations on the chest organs
Clinical manifestations:
- Chest pain that gets worse with breathing and coughing
- Dyspnea
- Palpable soft tissue thickening of the chest wall
- Crepitus on palpation due to blood clots
Diagnosis is based on history, clinical picture and imaging (radiography, ultrasound, CT).
Treatment depending on the size of the hematoma: puncture and drainage; with a large volume - thoracotomy and removal of clots.
Thus, extrapleural hematoma is a dangerous complication of chest injuries, requiring timely diagnosis and adequate treatment.
