Cholangitis Intrahepatic Primary

Primary intrahepatic cholangitis is an inflammation of the intrahepatic bile ducts not associated with mechanical obstruction.

Causes:

  1. Infections (viral, bacterial, fungal)
  2. Autoimmune diseases (primary biliary cirrhosis)
  3. Toxins
  4. Ischemia

Symptoms:

  1. Jaundice
  2. Pain in the right hypochondrium
  3. Fever
  4. Itchy skin

Diagnostics:

  1. Blood tests (elevated liver enzymes)
  2. Imaging (ultrasound, MRI, ERCP)

Treatment:

  1. Antibiotics
  2. Corticosteroids
  3. Choleretic drugs
  4. In case of cirrhosis - liver transplantation

The prognosis with timely treatment is favorable. In advanced stages, liver cirrhosis may develop.

Primary intrahepatic cholangitis (CICH) is an independent disease of the biliary tract, which develops in most cases without provoking external factors. It is characterized by frequent inflammatory lesions of both the proximal and distal parts of the biliary tract (duodenum, bile duct), as well as a reactive decrease in the body's immune system. If the mucous membrane of the gallbladder is included in the pathological process, then this is cholelithiasis of intrahepatic primary origin (CGD).

_1. Causes_

The cause of CIPP is identified by the anatomical features of the bile duct, which contribute to chronic disruption of the drainage function of the bile ducts as a result of: * hypokinesia of the biliary system; * cholestasis; * dyskinesia of bile and pancreatic ducts. * Functional changes such as a violation of the contractility of the gallbladder and the excretory section of the biliary system. In the process of inflammation as a systemic syndrome, the wall of the bile ducts is blocked: * morphofunctional obstruction caused by cholesterol and pigment stones, fibrous tissue changes, ctenic scars and dilation of the bile ducts. 2. Pathogenesis

In the pathogenesis of CIPP, as in any chronic pathology, lies dysmetabolic syndrome, against the background of which a decrease in resistance to the action of pathogenic microbes: bacteria or viruses develops, which leads to an infectious-inflammatory process (a diagram of impaired immune status in humans can be found HERE.).
