Gastroenterostomy Vertical

Gastroenterostomy is a surgical procedure that involves creating a permanent channel connecting the intestinal lumen and the external environment. As a result of such an operation, a closed system is formed in which intestinal contents continuously leave the body through the formed anastomosis, and are constantly replaced by new ones from the intestinal lumen.

Gastroenterostomies are of two types - simple and formative. A simple stoma for all operations begins with the formation of an outlet and an entrance anastomosis. The skin and soft tissue around the anus are first dissected. Then a hole 3-5 cm long is created in the intestinal wall, through which the folds of the mucous membrane of the small intestine are consistently turned out, and then the ligaments, muscles and peritoneum are cut out and pulled upward by the trunk. Forming a stoma is also a preparation of the intestines for surgery - so that applying a bandage to the suture does not interfere with the flow of contents out. The length of the forming stoma is 20-40 cm, since the mucous membrane is cut out in a single piece along with the mesentery. The operation allows you to form an anastomosis for any part of the intestine and provides convenient access to the surgical field, regardless of the distance from the navel. The location of the stoma can be either hepatic, that is, in the middle of the abdominal wall, or umbilical - on the abdominal wall opposite the navel, sometimes nasogastric and gastroduodenal - in areas adjacent to the stomach and duodenum. One of the advantages of gastroduodenostomy is that with this form, withdrawal can be carried out without the first bending of the wall or anastomosis. The procedure for forming such a stoma is possible if it is carried out according to a planned scheme and is accompanied by preliminary removal of the tumor from the stomach and subsequent plastic surgery of the stomach wall so that its edges can fit tightly to each other. The indication for such an operation is resection of other organs with neoplasms in the stomach, pancreas,
